個人資料蒐集、處理、利用同意書
國立臺灣文學館(以下簡稱本館)為遵守個人資料保護法規定,並保障當事人之權利,謹依法告知下列事項,請您詳閱。
一、蒐集個人資料之目的
本館為辦理各項推廣活動等相關業務,需蒐集活動報名者資料以便聯繫與通知。
二、蒐集個人資料之類別
姓名、電子信箱、行動電話。
三、個人資料利用之期間、地區、對象與方式
期間:活動報名日起至活動結束後3個月內。
地區:中華民國境內。
對象:由本館自行利用。
方式:本館將透過數位檔案形式蒐集、處理及利用您的個人資料。
四、個人資料之提供
您可自由選擇是否提供相關個人資料,惟若拒絕提供個人資料,本館將無法提供相關服務。依各項服務需求提供您本人正確、最新及完整的個人資料,若您的個人資料有任何異動,請主動向本館申請更正。若您提供錯誤、過時、不完整或具誤導性的資料,而損及您的相關權益,本館將不負相關賠償責任。填寫內容如有不雅,違反社會善良風俗,本館有權不予刊登或將您本人列入拒絕往來用戶名單。
五、個人資料之保密
本館將善盡個人資料保護之責,非經您本人同意不會自行修改、刪除或提供任何(或部份)個人資料及檔案。如因天災、事變或其他不可抗力所致者,致您的個人資料被竊取、洩漏、竄改、遭其他侵害者,本館將於查明後以電話、電子郵件或網站公告等方法,擇適當方式通知您。
六、當事人就個人資料得行使之權利
您可依個資法第三條規定,就本館保有您的個人資料行使以下權利:
請求查詢或閱覽。
請求製給複製本。
請求補充或更正。
請求停止蒐集、處理、利用或請求刪除,惟因本館執行業務所必須者,本館得不依請求為之。

Consent Form for the Collection, Processing, and Use of Personal Data
The National Museum of Taiwan Literature (hereinafter referred to as “the Museum”), in compliance with the Personal Data Protection Act and for the protection of your rights, hereby provides the following information for your review. Please read it carefully.
1. Purpose of Collection
The Museum collects personal data from event registrants for the purpose of organizing promotional activities and related operations, as well as for communication and notification purposes.
2. Categories of Data Collected
Name,, email address, and mobile number.
3. Period, Territory, Recipients, and Methods of Use
- Period: From the date of registration until three (3) months after the conclusion of the event.
- Territory: Within the territory of the Republic of China (Taiwan).
- Recipients: Internal use by the Museum only.
- Methods: Your personal data will be collected, processed, and used in digital file format.
4. Provision of Personal Data
Providing personal data is voluntary; however, if you choose not to provide the required information, the Museum may be unable to offer the related services. You must provide accurate, up-to-date, and complete information as required for each service. If your personal data changes, please inform the Museum to make corrections. The Museum is not liable for any loss of rights or interests resulting from inaccurate, outdated, incomplete, or misleading information provided by you. Any inappropriate or offensive content, or content violating public morals, will not be published, and the individual concerned may be listed as a restricted user.
5. Confidentiality
The Museum will take all necessary measures to safeguard your personal data and will not modify, delete, or disclose it—either in whole or in part—without your consent, unless required by law. In the event of theft, leakage, alteration, or other unauthorized access to your personal data caused by natural disasters, incidents, or other force majeure events, the Museum will notify you by telephone, email, or website announcement once confirmed.
6. Your Rights Regarding Personal Data
In accordance with Article 3 of the Personal Data Protection Act, you have the right to:
- Request access to or a copy of your personal data.
- Request supplementation or correction of your personal data.
- Request cessation of collection, processing, or use, or request deletion of your personal data. However, the Museum may refuse such requests if the data is necessary for its operations.
