1. 參加本次拍刺工作坊，需在上方連結報名。

To join this tattoo workshop, please register through the link above.

2. 錄取者之紋身體驗不收取費用，但為了保障彼此權益，請閱讀本表單內容和說明影片，確保本人理解並同意活動條件。

The tattoo workshop for selected participants is free of charge. However, to protect the rights of all parties, please read the notes and watch the video in this form carefully to ensure that you fully understand and agree to the event terms and conditions.

3. 受紋身者不得指定紋身師、圖紋、尺寸及部位。圖紋、尺寸及部位由紋身師進行設計，經與受紋身者討論同意後進行。每場紋身時間約 1-2 小時。

Participants do not have the option to choose their tattooist, design, size, or placement. The design, size, and placement will be determined by the tattooist in consultation with the participant prior to the session. Each tattoo session will take approximately 1–2 hours.

4. 受紋身者需接受館方安排 10/30 (五) 與紋身師進行現場互動與交談，在確認身心條件適合且填妥同意書及影像授權書後始得進行紋身。請注意，若無法參與 10/30 (五) 的互動交談，將不得進行紋身，請勿報名，敬請見諒。

Participants are required to attend an in-person interaction and consultation with the tattooists on 10/30 (Fri). Tattoo sessions may only proceed after confirming that the participant is physically and mentally suitable, and after all media consent and release forms have been completed and signed. Please note that participants who are unable to attend the interaction and consultation on 10/30 (Fri) will not be eligible for the tattoo workshop. Please do not apply if you are unable to participate. Thank you for your understanding.

5. 拍刺現場將進行影音拍攝及專訪，相關素材將作為本館未來研究及展示之用，確認錄取之受紋身者需於活動進行前簽署拍攝同意書。

Audio-visual documentation and interviews will be conducted during the tattoo sessions. The collected materials may be used for NMP's future research, publication and exhibition purposes. Selected participants will be required to sign media consent and release forms prior to the event.
6. 活動將於 09/22 (二) 17:00 截止報名。10/06 (二) 將以電子郵件通知錄取結果。若錄取者無法參與，將由備取名單優先候補，或開放現場民眾參與。

Application will close at 17:00 on September 22 (Tue). Admission results will be notified via email on October 6 (Tue). If selected participants are unable to attend, candidates on the waitlist will be given priority for replacement, or participation may be opened to walk-in attendees on-site.
7. 史前館保留修正、暫停、終止或解釋本活動之最終權利。
NMP reserves the right to amend, suspend, terminate, or interpret the event at its sole discretion.

8. 本表單所蒐集之個人資料僅用於聯繫報名者，並提供紋身師了解報名者狀況，不轉做其他用途，亦不會公佈任何資訊，資料將於活動結束後銷毀。

The personal information collected in this form will be used only for communication with applicants and to help the tattooists in understanding participants' conditions. The data will not be used for any other purposes, will not be disclosed publicly, and will be securely deleted after the event concludes.
